    To realize the great rejuvenation of the Chinese nation and to build an innovative country is both a historical requirement and a mission of the times. Education is a national priority. How to establish an effective mechanism for training top-notch innovative talents and how to promote them to come to the fore are urgent requirements for current educational reform. To strengthen the selection and training of top-notch innovative talents of basic disciplines,and serve the countrys major strategic needs, the basic medicine specialty of Zhongshan Medical College, Sun Yat-sen University has continuously promoted the work deployment of the Ministry of Education in recent years, which takes “four orientations” as the starting point of talents training, relies on abundant resources and conditions for running schools, promotes the “Double First-Class” development, implements the “Double Ten-Thousand Plan” and the “Plan for Strengthening Basic Academic Disciplines”, builds the top-notch base, and leads teachers and students to focus on the worlds scientific and technological frontiers, major national strategies and peoples life and health needs to cultivate top-notch innovative talents for serving the national biosafety strategy and meeting major human health challenges, and so to explore and practice the training mechanisms of top-notch innovative talents in basic medicine.

    The training mode of top-notch innovative talents in basic medicine is aimed at exploring the cultivation outstanding talents under the condition of mass higher education. Internationalization is an important part of the training mode of top-notch innovative talents in basic medicine. We extensively draw on advanced experience at home and abroad, rely on various cooperative projects to cultivate students international vision, carry out characteristic activities to cultivate students humanistic feelings and sense of responsibility, and achieve certain results in talents’ training mode. In the process of exploration and reform, through extensive investigation, we have identified the importance of internationalization for training top-notch innovative talents, and will continue to improve the training mode to contribute to the training of top-notch innovative talents.

    The dual tutorial system has been implemented at Zhongshan Medical College, Sun Yat-sen University for undergraduates in basic medicine for many years. It refers to general tutors starting from the second to third academic years and scientific research tutors in the fourth to fifth academic years. The general tutors guide and help students in professional thinking, professional learning, scientific research training and career planning, and train students in basic scientific research training, literature review ability, etc.; while the scientific research tutors are responsible for the guidance of students graduation design, research project implementation, and thesis defense and so on. Through regular selection of excellent tutors, organization of tutor-student meetings, encouraging students to enter the tutor team early, enter the laboratory early, early scientific research projects and other ways, has formed a high-quality tutor team, a number of national student research awards, effectively enhance the students’ interest in scientific research, innovation ability, which had been unanimously praised by teachers and students of the college. Early contact with scientific research has become an important part of our school’s and three “earlies” medical education tradition, and tutorial system has also become an important brand in the undergraduate education and teaching stage of our school. Zhongshan Medical College, Sun Yat-sen University obtained the first batch of strong foundation plan projects in basic medicine specialty in 2020, the tradition has been inherited and developed, and solidified into the content of the training program for basic medicine professionals, which has become one of the powerful measures to promote the implementation of the plan for strengthening basic academic disciplines. This article focuses on how to further develop undergraduate tutorial system in basic medical disciplines combined with the requirements of strong foundation and top-notch talents training.

    The development of modern medicine and society puts forward higher requirements for the training mode of basic medical talents. Certain common problems exist in the experimental teaching of basic medicine in domestic colleges and universities, which are mainly manifested in the separation of teachers from students, the separation of basic medicine from clinical medicine, the separation of theory from practice, as well as the separation of teaching from scientific research. In recent years, Zhongshan Medical College, Sun Yat-sen University has actively promoted the experimental teaching reform of basic medicine for cultivation of top-notch innovative talents, and explored an integrated experimental teaching mode of basic medicine combining “first class” (experimental teaching in classroom) and “second class” (research practice in principal investigator laboratory). Through various teaching modules in “first class”, including comprehensive experiments, seminars and biosafety virtual experimental teaching, students can understand the corresponding frontier technology and scientific research progress. Meanwhile,through the research practice of PI laboratory in the “second class”, students can get the “one-to-one” guidance from scientific research tutors and systematic standardized scientific research training during the whole process, as well as complete innovative experimental design and opening defense. After five years of practice, this integrated experimental teaching mode of “dual class”in basic medicine is helpful to improve students autonomous learning ability, innovative thinking and scientific research practice ability, and therefore provides useful exploration and reference for the cultivation of top-notch talents in basic medicine.

    General education is an important part of higher education,  and responsible for improving the comprehensive quality of college students. Compared with comprehensive universities,  the elective courses offered in independent medical schools are too specialized and not rich enough. How to optimize the knowledge structure of medical students and improve their scientific literacy and humanistic quality has been an important issue in the education and teaching reform of independent medical schools. Through a systematic analysis of six consecutive semesters of sub-modular general education courses at Capital Medical University,  we intend to put forward rationalized suggestions for independent medical schools to further utilize the advantages of online general education courses and improve and optimize the online general education course system.

    Taking the neurorehabilitation course offered to medical technology (rehabilitation therapy) majors in Chongqing Medical University as an example,  this study explains the integration of ideological and political education and specialty teaching in the new era of medical technology education. On the basis of learning pyramid theory,  following the laws of medical education and the talents training goals of rehabilitation therapy,  combining a variety of classroom styles,  setting a progressive goal of ideological and political education,  we offer a novel model of ideological and political theories in curriculum teaching. Then we explore the “One Center,  Two Motivations,  Three Combinations,  and Four Integrations” mode so as to effectively strengthen the construction of first-class discipline and curriculum,  and cultivate these new medical rehabilitation therapists who have “five techniques” in full-cycle of prevention,  treatment and wellness.

    Ideological and political education is a new requirement for molecular biology teaching in medical universities in the new era. In teaching Traditional Chinese Medicine (TCM) majors,  based on the concept of inheritance and innovation,  we explored the traditional culture of TCM,  fully integrated ideological and political elements into the curriculum system construction. The depth and breadth of ideological and political education was expanded in the process of teaching objectives,  content,  methods and evaluation,  and we set up a comprehensive curriculum ideological and political system of molecular biology. Ideological and political education runs through the whole process of molecular biology teaching,  enhances the mission sense of inheriting the essence among TCM majors,  keeping their integrity and innovation,  and fulfilling the fundamental task of strengthening moral education and cultivating  people. It provides useful experience for the ideological and political teaching reform of molecular biology courses in TCM specialty.

    Ideological and political education in curriculum is a comprehensive educational concept that organically integrates the elements of ideological and political education into the whole process of specialty teaching,  as well as an important way to implement moral education. Our survey found weak awareness of rural oriented medical students to voluntarily serve grass-roots medical institutions,  and low renewal rate in grass-roots institutions after the expiration of service. This study discusses the path of ideological and political education in curriculum for rural oriented medical students in the new era from the aspects of teachers' ideological and political literacy,  the exemplary role of teachers' role models,  the establishment of curriculum ideological and political demonstration courses,  the construction of curriculum ideological and political evaluation system and support from school. Rural oriented medical students should set up correct outlook on life and values,  we should improve their awareness to serve the grassroots,  and train grassroots health talents who are “ready to go,  happy to stay and good to help”,  so as to help the construction of “Healthy China”.

    In order to implement the State Council’s “Overall Plan for Deepening Educational Evaluation Reform in the New Era”,  to improve the system and mechanisms of moral education and cultivation,  to reform evaluation standard for evaluating clinical teachers’ teaching ability,  and to promote the implementation of the mission of imparting knowledge and education,  we established and took the lead in practicing a teaching performance evaluation system for clinical teachers according to their teaching content and characteristics at Capital Medical University. The aim of our comprehensive teaching ability evaluation index system is to carry out quantitative and scientific classification of comprehensive teaching ability evaluation. From November 2020 to January 2021,  we performed the first-time comprehensive teaching ability evaluation for a total of 7, 680 clinical teachers from 34 teaching units including various clinical medical schools and clinical teaching hospitals. Through systematic evaluation and analysis,  we confirmed the average score of comprehensive teaching ability for teaching professors and associate professors. This work provides an important reference for the evaluation of teaching-oriented clinical teachers,  which was used to determine their suitability for promotion at our university in November 2021. This study provides new ideas for educational evaluation reform based on practical experience for the current generation of clinical teachers in China.

    “Double First-Class” university construction is another key construction project in China’s higher education. To strengthen educational system reform and effectively improve teachers’ teaching level is not only the key and guarantee to improve higher education quality in the new era,  but also the inherent inevitable requirements of the “Double First-Class” universities construction. During this new historical period,  teachers’ teaching level development faces new challenges.     The synergy of health care system and medical educational system is an important task to deepen medical education reform and an important guarantee to build the strategy of “Healthy China”. With the advancement of the synergy of health care system and medical educational system,  China's higher medical education has carried out the reform of “5+3” medical education model,  and implemented the integration of professional degree postgraduate education and standardized training of residents. Meanwhile,  the professional degree postgraduates who participate in the standardized training of residents show the lack of scientific research ability,  but the traditional “single tutor system” may not meet the needs of training more innovation-oriented talents. The “double tutor system” may make up for this limitation and help improve the quality of medical personnel training,  which is an important educational innovation to deepen the synergy of health care system and medical educational system.     The outbreak of COVID-19 and the normalized epidemic prevention have different effects on postgraduates’ study,  graduation and employment,  resulting in their great anxiety and irritability. This paper analyzes,  summarizes the problems of the difficult management of nosocomial infection prevention and control and the problems of students’ anxiety during this outbreak of COVID-19.     Objective  To explore the application of problem-based learning (PBL) combined with 3D-slicer image reconstruction technology in neurosurgical clinical practice teaching for long-term-system medical students.  Methods  A total of 62 long-term-system medical students underwent neurosurgical clinical practice teaching in our department from April 2019 to June 2019 were selected. They were randomly divided into experimental group and control group. The control group received traditional teaching mode,  while the experimental group was given PBL teaching method combined with 3D-slicer image reconstruction mode. The teaching effect was evaluated by   clinical practice test,  theory examination, mini-clinical evaluation exercise (Mini-CEX) and student questionnaire. Results   The clinical practice test results,  theoretical assessment results and Mini-CEX results (all P<0.05) in experimental group were significantly higher than those in control group. According to the questionnaire survey,  students in experimental group were more satisfied with the class atmosphere,  radiological image evaluation,  operation explanation and satisfaction of teaching than those in control group ( all P<0.05). Conclusion  In the neurosurgical clinical practice teaching in long-term-system medical students,  PBL teaching method combined with 3D-SLicer image reconstruction technology can stimulate students' interest and initiative in neurosurgical practice learning,  improve the learning efficiency and the teaching effect.

    Pediatric intensive care medicine (PICU) is the interdisciplinary and professionalized major. The situation of patients in PICU are varied and complex. Training pediatrician is critical thing. Participatory teaching method is student oriented. It could not only enhance the initiative and motivation of medicine but also cultivate their self-study ability. Moreover it helps to provoke the teacher’s ability. So it is useful for pediatric intensive medicine teaching.     Objective  To analyze the spatial accessibility of tertiary hospitals in Tongzhou District of Beijing before and after the relief of non-capital functions,  so as to provide reference for further optimizing the spatial layout of medical resources.  Methods  We collected the resource allocation data of tertiary hospitals and population distribution data in Tongzhou District before and after the relief of non-capital functions,  and analyzed the spatial accessibility of tertiary hospitals in Tongzhou District by comprehensively using the methods of service area analysis,  spatial accessibility analysis and local spatial autocorrelation analysis based on geographic information system (GIS). Results   The population and tertiary hospitals in Tongzhou District are mainly distributed in the 155 km2 area,  also called the city sub-center. From 2020 to 2025,  the spatial accessibility of tertiary hospitals has changed from single core radiation to dual core radiation,  and the overall level and equality have improved. By 2025,  the 30-minute-service area of tertiary hospitals will cover 94.39% of the residents in Tongzhou District. The allocation level of tertiary medical resources in the city sub-center and surrounding areas is higher than the average level of the whole region. There is a degree of tertiary medical resource aggregation in the city sub-center and surrounding areas. Conclusion  After the relief of non-capital functions,  the level of tertiary medical resource allocation in Tongzhou District makes it convenient for most residents to enjoy high-quality medical services. The centralized layout of tertiary medical resources meets the needs of city sub-center construction and “two districts” construction.

    Objective  To improve family doctor team home-visiting service system based on Beijing family doctor team members’ opinions of the home-visiting conditions,  personnel,  procedures and other aspects.  Methods  A questionnaire survey was conducted among family doctor team members in some community hospitals in three urban areas of Beijing. Results   General practitioners,  nurses and family doctor assistants had the same cognition. In terms of home-visiting preconditions,  39.45% and 54.13% disagreed with the restriction of patients age and disease type; and 60.55% believed that only patients with severe disability or above could enjoy the home-visiting service. In terms of home-visiting staff,  65.14% of the members agreed that one doctor and one nurse should team up for home visits. In terms of visiting procedures,  43.12% members agreed with the process of “patient application,  family doctor review and evaluation,  community hospital approval of visiting”. Conclusion  It is suggested that relevant government departments should clarify the preconditions for home-visiting patients and focus on taking care of patients over 70 years old who suffer from chronic diseases and severe disabilities. The allocation of home-visiting medical staff shall be determined according to the diagnosis and treatment catalogue and procedure for home-visiting should be standardized and simplified.
    Ending compulsory medical treatment of patients with mental disorders under the background of risk prevention—based on the cognition survey of community prevention psychiatrists

    Objective  To understand the problems existing in ending of the compulsory medical treatment system of mentally disabled patients under the background of risk prevention,  and to provide corresponding improvement suggestions.  Methods  We conducted a questionnaire survey among psychiatrists randomly selected from community health service centers in  four districts in Beijing. Multiple response analysis and Chi-square goodness-of-fit test were performed using SPSS 25.0. Results   A total of 136 (63.6%) of the psychiatrists believed that the Ankang Hospital should apply for the ending compulsory medical treatment,  and only 9 (4.2%) of them thought that the patients should apply by themselves. For the decision subject of ending compulsory medical treatment,  psychiatrists who chose Ankang Hospital and Judicial Accreditation Agency are both 73 (34.1%),  while only 18 (8.4%) chose the court. Comparison of the reasons why “the criminal compulsory medical treatment is difficult to end” were statistically different (χ2=12.243,  P=0.016). Psychiatrists who chose “unreasonable standards” reached up to 151 (70.6%). For patients' home management problems,  88 (41.1%) of the psychiatrists chose “health departments and public security departments cooperate with each other”. The proportions of psychiatrists choosing different problems existing in “connecting between ending of criminal compulsory medical treatment and community management” were not statistically different (χ2=2.103,  P=0.551). Conclusion  The implementation of ending of the compulsory medical treatment system concerning the entities of application,  release standard,  trial mode and connection with community management has imperfections. It is advisable to improve the regular evaluation system,  to make up for the cracks between medical and legal standards,  and to complete the comprehensive evaluation release standard. We should refine the opening trial procedure of compulsory medical treatment release and include patients into the scope of community rehabilitation system. The problem of “the compulsory medical treatment is hard to end” can be solved by community rehabilitation system,  and the patients can get effective treatment and return to society.

    Objective  To evaluate the doctor-patient communication ability of standardized training residents,  to analyze the problems existing in the cultivation of doctor-patient communication ability,  and to provide the basis for further improvement in the process of standardized residents training.  Methods  The SEGUE scale was used to evaluate the doctor-patient communication ability of 79 residents in 2019 from five dimensions of “set the stage”,  “elicit information”,  “give information”,  “understand patients perspective”,  and “end the encounter”. SPSS23.0 statistical software was used to compare the score rate of each index between groups by chi-square test and multiple comparison. Results   The full score of residents doctor-patient communication ability is 25,  and the average score of residents in this project is 16.58 ± 1.80.The score rates of “set the stage”,  “elicit information”,  “give information”,  “understand patients perspective”,  and “end the encounter” are 67.80%,  72.90%,  49.00%,  91.75%,  and 13.50% respectively. There are significant differences among the secondary indicators in the four primary indicators of “set the stage”,  “elicit information”,  “give information”,  and “understand patients perspective”(P<0.001,  P<0.001,  P<0.001,  P=0.017),  and there are gender differences in the scores of some secondary indicators. Conclusion  It is necessary to further strengthen the systematic training of residents’ doctor-patient communication ability. The traditional examination consists of completion assessment,  annual examination and graduation examination,  overemphasizing theory test and skill examination,  so examination of doctor-patient communication ability should be increased,  aiming to ensure and improve the quality of residents’ post competency training.

    Qualitative study on promoting and hindering factors of nursing undergraduates participation in National Medical Technical Skills Competition for College Students

    Objective  To explore nursing undergraduates’ perceptions,  attitudes,  and experiences when preparing for National Medical Technical Skills Competition for College Students,  to summarize promoting and hindering factors in participation in this competition,  and to provide reference for nursing students to take part in future event.  Methods  We conducted semi-structure interviews to collect the data of 14 participants from four schools in Jiangsu,  Jiangxi and Guangxi provinces. The manuscripts were transcribed and analyzed with Colaizzi’s 7-step analysis. Results   We gained insight into the promoting and hindering factors of students’ competition participation. The promoting factors include “the improvement of individuals’ ability”,  “the development of career”,  and “the improvement of social relationships”. The hindering factors were comprised of “school’s indifference to the competition”,  “teachers’ inadequate preparation”,  “personal factors” and “the high demands and challenges of the competition”. Conclusion  Nursing schools should pay more attention to and publicize the national competition and promote the construction of high-level instructor teams. Additionally,  it is critically important to focus on and coordinate the preparation arrangement of teachers and nursing students in order to facilitate participation and to improve the performance.

    Generalized anxiety disorder is a pathological anxiety characterized by persistent nervousness accompanied by significant nervous function excitement and excessive vigilance. Its psychological symptoms,  physical symptoms and behavioral manifestations seriously affect college students’ daily life,  study and development,  and impose severe pain and burden on individual students and their families. Because traditional drug therapy is prone to drug tolerance,  drug dependence and repeated adverse conditions after drug withdrawal,  psychological therapy plays an important role. This paper was based on the appearance-verve theory and seven emotions theory in Traditional Chinese Medicine(TCM),  on copious literature review research combined with case analysis,  then put forward generalized anxiety disorder psychological intervention measures,  with “persuasion channel,  modern phase,  empathy is easy” as its core. It advocates mind therapy,  situational therapy,  desensitization therapy for college campus prevention of generalized anxiety disorder. It also provides reference for college students to carry out psychological counseling and mental health education.
