    Regional practice and suggestions for the standardized training of residents: based on Shanxi Province

    To understand the status and influencing factors of the implementation of the “Unification of Four Certificates” training program for clinical medical postgraduates in Shanxi Province, a self-designed questionnaire was adopted to investigate and analyze the basic information of Grade 2015 clinical postgraduates in Shanxi Province, the basic conditions of training, the effectiveness of clinical rotation, teacher training, existing problems and solutions. The Cronbach’s α reliability coefficient of the questionnaire was 0.89. The structural efficiency analysis showed that the KMO test value was 0.93, the Bartlett spherical test value was P<0.01, and the cumulative variance contribution rate was 64%. Results showed that 70.63% of the respondents chose “out of future employment consideration” as the reason for their participation in the “Unification of Four Certificates.”46.39% of postgraduates thought they had “considerable pressure” since they took part in standardized training. Most of the pressure came from “publication pressure”. 34.97% of people thought they had “more” clinical practice opportunities. At present,the system of the “Unification of Four Certificates” in Shanxi Province has been improved. The awareness and satisfaction of trainees are generally good, but there are many weak links, such as the arrangements of clinical rotation and the training modes.Hopefully, this investigation can provide reference for improving the future residents training system.

    The standardized training of residents serves as the bridge and foundation in training excellent medical professionals, and an important measure in improving the comprehensive quality of clinicians. We undertakes the standardized training of residents and professional postgraduates, aiming to train surgeons with noble medical ethics, super medical skills and systematic clinical thinking ability.     This article aims to explore the reform of the post competence oriented teaching method in standardized training of neurological residents. The neurological training programs were established with specialty features. A series of strategies were discussed, focusing on medical humanities accomplishment, educating professional ethics, strengthening effective doctor-patient communication skills training, cultivating the independent learning ability. It would help residents to improve medical service and to be a high-quality neurologist under the new medical situation.

    Objective To explore the teaching effect of clinical pathway problem guidance in standardized training of cardiological residents. Methods From March 2016 to March 2018, we enrolled 72 residents in Department of Cardiology of the Pangang General Hospital of Kunming University of Science and Technology. According to the randomization principle, they were divided into experimental group (38 trainees accept the clinical pathway problem guidance method) and control group (34 trainees accept the clinical teaching of traditional standardized training of residents). The trainees were trained in the Department of Cardiology for 2 months. After training, we evaluated the teaching effect by analyzing objective and subjective assessments. Results The basic theoretical knowledge in the experimental group was higher than that in the control group [(46.3±3.1) vs. (41.7±2.9)]. The case analysis in the experimental group (47.7±2.1) was higher than that in the control group (40.2±3.4). The skill operations in the experimental group (46.8±2.7) was higher than that in the control group (39.6±4.9). The difference of the two groups was statistically significant (P<0.05). In terms of arousing learning interest, improving the independent thinking, learning ability, case analysis ability and clinical skill operations,the experimental group were higher than the control group respectively (81.6%vs.8.8%), (92.1%vs.2.9%), (89.5%vs.5.9%), (84.2% vs.8.8%), (81.6% vs.8.8%). The difference of the two groups was statistically significant (P<0.05). Conclusion The clinical pathway problem guidance method can effectively improve teaching quality of standardized training of cardiological residents.

    In order to explore the ways of employment guidance based on WeChat public platform in medical universities,this article is based on the practice of WeChat public platform construction in employment guidance of graduated students in our university from 2015 to 2017, together with satisfaction analysis by questionnaire method. WeChat public platform could improve the situation of employment guidance in our medical university and provide effective information. In the future, it would enrich the content and form of information, especially focus on the information of employing unit outside Beijing and grass-roots units, enhance the coverage of information, provide more effective employment guidance for graduates.

    In order to achieve the training goal of “top-notch innovative talents”, the Ministry of Education attaches great importance to the reform of the doctoral admission system. In recent years, the doctoral application for examination and approval is a hot topic of reform. Capital Medical University, after thorough survey, started this reform in 2017 among a part of tutors and explored the doctoral application review system. This article introduced the experience of Xuanwu Hospital and pointed out the issues concerning the publicity, the key points of the application, how to avoid the power rent seeking phenomenon, the problems of coexistence of different recruit systems etc. The main purpose of this article is to explore how to perfect this system and make it precise and executable.

    Objective to understand the implementation of the reform of graduate training system in surgery and analyze the existing problems. Methods 38 graduate tutors and 43 graduate students were selected from Beijing Chaoyang Hospital affiliated to Capital Medical University and were investigated by questionnaires, and the results were analyzed.Results The tutors and postgraduates agreed that the abilities of the postgraduates in surgery were above the average, and the scientific research training was of great help for the future clinic. However, 60% of the postgraduate students had not received the scientific research training. The communication between tutors and postgraduates are reduced and most tutors argued that the new model weakened the specialized training of postgraduates. Conclusion The training quality of the postgraduate students in the new medical master degree training model is higher, but there is still space for improvement, and the cultivation of clinical ability and scientific research ability of the clinical medical degree graduate students are combined to promote each other. Under the new training mode, the communication between tutors and graduates student is reduced,and the supervision of tutors is decreased. The graduate student training shows the trend of major surgery. Most graduate students do not have time for comprehensive professional training and preparations.

    This study collected and analyzed data on the clinical practice ability of standardized training of psychiatric residents in Beijing psychiatric hospitals from 2014 to 2016. The statistical method was used to test the validity and reliability of clinical practice ability assessment program and to provide reference for the future standard norms of the assessment mode. The problems exposed in the assessment of residents training was addressed and practical measures was put forward to improve the quality of resident training programs.

    Objective To study the effect of step-by-step training mode in standardized training of residents in ultrasound specialty, and to provide basis for standardized training of residents. Methods The residents who completed the training and examination in Department of Vascular Ultrasound, Xuanwu Hospital from 2011 to 2017 were continuously included. In 2011-2014 years, the traditional “imparting” teaching method was used as the control group. In the 2015 -2017 years, the “step-by-step” training mode was used as the experimental group. The pass rate of theoretical test and skill test of the two groups were compared in the first stage of residents in Beijing. Result In totality, 28 residents were included in the current study, namely, 14 in the control group and 16 in the experimental group (2 of them did not passed the 2014 examination). The pass rate of the theoretical assessment was 100% and 93.75% in the experimental group and the control group, respectively (χ2=1.096, P=0.485); the pass rate of the skill assessment was 94.12% and 47.37%, respectively. The step-by-step training mode improved the pass rate of skill assessment significantly, and there was a significant difference (χ2=12.389, P<0.001). Conclusion The effect of the step-by-step training mode is obviously better than that of the traditional training, and it can be further applied in the training of resident doctors in ultrasonic specialty.

    Clinical practice is an important practical process that modern medical students must undergo before they legally practice medicine. At present, there are some problems such as teaching resources imbalance between supply and demand, outmoded teaching concept and incomplete assessment feedback mechanism. This article explored the reform of clinical teaching, basing on mature clinical pathway information system, as well as focusing on “standardized medical order” of clinical common diseases in clinical teaching. It will be of some use in homogeneous clinical practice to sum up the new teaching experience, to improve the efficiency of clinical teaching, and to ensure homogeneous clinical practice can provide the beneficial reference.

    Objective To understand the current situation and existing problems of the teaching mode of pediatric dermatology, and to explore the teaching concept and teaching effect of the innovative mode. Methods By comparing the new and old teaching modes, the problem-and case-based learning teaching method, the existing traditional teaching mode, the multimedia teaching and the network teaching method are organically combined, and they are applied in the basic and clinical teaching of the pediatric dermatology department. Results The innovation in the teaching mode of pediatric dermatology can effectively stimulate students’ learning initiative, improve their clinical skills and enhance the effect of learning, so as to improve the teaching quality. Conclusion The teaching mode of diversification as the core, the teaching mode focusing on clinical practice and the teaching mode of the comprehensive use of the Internet plus as the platform are introduced into the teaching of pediatric dermatology, which greatly improves the students’ practical ability to solve the problems and promotes the teaching of pediatric dermatology.

    Objective Based on the (comprehensive) ranking and scoring, the index system and the corresponding data from the 2017 Sci-tech Influence Evaluation Index System for Hospitals in China, this study aims to analyze the status quo of the sci-tech influence of the Top 100 municipal tertiary hospitals in Beijing, and to provide strategic suggestions for municipal hospitals to reinforce their sci-tech influence. Methods The descriptive statistical analysis was used. The input,the output, the influence and the scientific research efficiency of Beijing municipal hospitals were analyzed and compared with the hospitals directly under the central government. Results Totally eight Beijing municipal tertiary hospitals were among the Top 100 nationally influential hospitals in China in 2017. In recent years, the overall scientific and technological strength has been increasingly improved. However, compared with the hospitals directly under the central government in Beijing, the number of Natural Science Foundation of China (NSFC) projects, the recognized treatises with high impact factors and the scientific research efficiency all need to be further promoted. Conclusion It is necessary to develop appropriate strategies and measures via integrating multiple indicators of the scientific and technological input, the scientific and technological output and the influence, etc., and to improve the scientific research efficiency, to strengthen the reserve of longitudinal scientific research projects, especially that of the NSFC projects, to upgrade the quality of papers and the patent outputs, in order to enhance the sci-tech influence of municipal hospitals.

    In order to deepen research of medical reform, Beijing Traditional Chinese Medicine Hospital affiliated to Capital Medical University appointed management personnel to operate and manage other traditional Chinese medicine medical institutions. The close cooperation is established between two organizations. The recipient’s management level and medical service ability have been improved. This paper conducted a questionnaire survey of patients’ satisfaction in three different levels of traditional Chinese medicine medical institutions under the custody, and concluded that patients’ satisfaction has improved compared with that before the custody. However, patients are still not satisfied with their diagnosis and treatment condition, waiting time, service attitude, medical facilities, medical environment and other aspects. The author hopes to continue to explore the custody form, deepen the classification diagnosis and treatment, improve the medical facilities, optimize the medical environment and other aspects of improvement, so as to provide basis and experience for further medical reform and effectively to realize the classification diagnosis and treatment.

    Objective To study the situation of cardiopulmonary resuscitation training and their related legal cognition in university students in Beijing, and to discuss the relevant countermeasure. Methods Stratified random sampling was adopted. Results A total of 882 students in 10 Beijing universities were investigated. Results showed that those surveyed had a low 22.3% participation rate of cardiopulmonary resuscitation training. The proportion of medical students participating in cardiopulmonary resuscitation training was relatively high, accounting for 35.5% of the students in this category. 89.6% students were willing or very willing to attend relevant training. While only 18 students knew the specific laws about protecting first aid providers, and 97.3% students favored the establishment of legal provisions to protect first aid providers.Conclusion The situation of cardiopulmonary resuscitation training for college students in capital universities and their related legal cognition didn’t meet the needs of the society. Only through popularization and standardization of the first aid training, increasing the number of emergency equipment such as AED in public places, speeding up the process of legislation to protect first aid providers, can the universities promote the dissemination of first aid knowledge, and make college students become the main force and core group of self-rescue and mutual aid.

    Objective to explore the current status and problems in the establishment management of Beijing municipal hospitals. Methods The interview method was used among the experts in the human resource management department of the municipal Grade III tertiary general hospitals. Results 6 major themes and 6 sub-themes were revealed through the analysis of interview content, including the preparation scale, management policy, knowledge of establishment management,existing problems and causes, etc. Conclusion Exploring the two government-led management models of intensive and open-type management can provide foundation for comprehensive reform goals of public hospitals in Beijing and the innovative preparation of management methods.
