教学门诊在住院医师规范化培训中的实践与探索

摘要: 目的 探讨教学门诊对住院医师临床能力的提升效果及实施效果。方法 选取2023年5月至2024年11月期间参加浙江省肿瘤医院教学门诊的全体带教师资及二、三年级住院医师作为研究对象。采用Mini-CEX量表对住院医师进行临床能力评估,并通过问卷调查分析教学门诊实施效果。结果 住院医师第2次Mini-CEX评估中各维度临床能力得分均高于第1次评估,差异具有统计学意义(P<0. 05)。师资调查问卷结果显示,带教老师认为教学门诊的主要难点是分层教学(68. 42%)和指导住院医师独立接诊(63. 16%),最难实现的教学目标是培养住院医师临床思维和决策能力(73. 68%),最需强化的内容是问诊技巧(78. 95%)。结论 教学门诊有助于提升住院医师各维度临床实践能力,仍需通过强化师资队伍建设、拓宽教学门诊患者招募途径、建立常态化反馈机制等方式提高教学门诊质量。

Abstract: Objective To explore the effect of teaching clinics on enhancing the clinical competence of residents and theirimplementation effect. Methods All standardized training instructors and postgraduate year (PGY) 2 and 3 residents whoparticipated in the teaching clinic of Zhejiang Cancer Hospital from May 2023 to November 2024 were selected as the studysubjects. The Mini-Clinical Evaluation Exercise (Mini-CEX) scale was used to evaluate the clinical competence ofresidents, and a questionnaire survey was conducted to analyze the implementation effect of teaching clinics. ResultsResidents' clinical competence scores in each dimension in the second Mini-CEX assessment were higher than those in thefirst assessment, with statistically significant differences (P<0. 05). The results of the faculty questionnaire showed that themain difficulties of teaching clinics identified by teachers were hierarchical teaching (68. 42%) and guiding residents toreceive patients independently (63. 16%). The most difficult teaching goal to achieve was cultivating residents' clinicalthinking and decision-making ability (73. 68%), and the content most in need of strengthening was inquiry skills (78. 95%).Conclusion Teaching clinics are helpful to improve residents' clinical practice abilities across all dimensions. However, itremains essential to improve the quality of these clinics by strengthening faculty development, broadening patientrecruitment strategies for teaching clinics, and establishing a regular feedback mechanism.
