首都医科大学临床医学课程标准化题库建设:技术参数体系的构建与应用

摘要: 目的 针对传统纸质化即时命题组卷考试存在规范化不足、考核评价客观性欠佳、试题管理不便等问题,依托计算机信息技术背景,探讨标准化题库建设对深化医学教学改革的路径与意义、适配医学教育发展及社会需求的价值,为医学院校标准化题库建设提供实践参考。方法 以首都医科大学临床医学课程标准化题库建设实践为研究基础,围绕标准化题库建设的意义、总体思路、设计方案进行顶层设计,组织实施数字化平台搭建,分阶段推进建设要点展开。结果 传统教师即时命题组卷的纸质化考试存在考核规范性欠缺、难以全面客观评价学生学习能力与教师教学效果、考试信效度不足、人力成本高、试题更新与管理不便等弊端;临床医学课程标准化题库采用题量规划(1:3)、题型比例(A1 25%)等技术参数,具备科学、规范、高效等多重优势。目前,首都医科大学已建成内、外、妇、儿标准化题库,并广泛应用于课程统考及临床综合考试。实践数据显示,题库的类型、题量、题型设计以及技术参数的设计较为合理,有效支撑了考教分离与无纸化考核的实施,显著提升了考核的信度与效度。结论 本文最大的创新在于提出了一套具体、量化、可复制的“技术参数体系”。标准化题库建设契合医学教育发展趋势,是考教分离的基础,有效地四方联动和动态维护能够有效弥补传统纸质化考试的短板,可为医学院校题库规范化建设提供实践借鉴。


Abstract:

Objective Addressing the shortcomings of traditional paper-based immediate question-setting and test administration, including insufficient standardization, poor objectivity in assessment, and inconvenient question management, this study explores the value of standardized question bank development in advancing medical education reform, aligning with educational advancements, and meeting social needs, against the backdrop of computer information technology. It aims to provide practical references for standardized item bank construction in medical universities.

Methods Drawing on the standardized question bank development experience for clinical medicine courses at Capital Medical University, this study conducted top-level design of standardized question banks regarding the significance, overall framework, design approach, implementation details, and advanced key construction tasks in phases .

Results Traditional paper-based exams face challenges including lack of assessment standardization, difficulty in comprehensively and objectively evaluating students' learning abilities and teachers' teaching effectiveness, insufficient examination reliability and validity, high labor costs, and inconvenient question updating and management. The standardized question bank for clinical medicine courses adopts technical parameters such as question quantity planning (1:3 ratio) and question type proportions (Type A1: 25%), demonstrating multiple advantages of being scientific, standardized, and efficient. Currently, Capital Medical University has established standardized question banks for internal medicine, surgery, obstetrics and gynecology, and pediatrics, which have been widely applied in unified course examinations and clinical comprehensive examinations. Practical data indicate that the question types, quantity, question type design, and technical parameter design of the question bank are reasonably structured, effectively supporting the implementation of the separation of teaching and examination as well as paperless assessment, and significantly improving the reliability and validity of assessments.

Conclusion The primary innovation lies in proposing a concrete, quantifiable, and replicable "technical parameter system." Standardized item bank construction aligns with the development trends of medical education and serves as the foundation for the separation of teaching and examination. Effective four-party collaboration and dynamic maintenance can effectively address the shortcomings of traditional paper-based examinations, providing practical experience for the standardized construction of question banks in medical universities.
